Glaucoma in South Korea

[vc_row][vc_column][vc_column_text]South Korea has been collecting epidemiological data regarding eye health from the Korean National Health and Nutrition Examination Survey (KNHANES) since 2008. A considerable amount of scientific articles have been published from this data about an ethnically homogeneous population with high myopia and prevalence of various eye diseases. The results of the survey emphasize the importance of an evidence-based approach while contributing to the awareness of eye health among the general population.

Access to eye health in South Korea is high. For instance, Cataract Surgical Rate (CSR), which is the primary indicator for the eye health accessibility of a country, has increased from 2,762 in 2005 to over 10,000 in 2017. However, awareness for glaucoma in the country is still low and so, we have not provided enough treatments.

From the KNHANES data, we have obtained plenty of new knowledge regarding the prevalence of glaucoma, and other retinal diseases. Starting from 2017, data regarding Humphrey perimetry and OCT were added to intraocular pressure (IOP) and fundus photos to measure the prevalence of glaucoma more accurately. The most recent data from the 2017 survey depict that 6.1% of the population is suspected to have glaucoma, 3.7% have ocular hypertension, and 3.3% have Primary Open Angle Glaucoma with most of them registering IOP of 21 or less.

The level of awareness, which is represented by the rate of glaucoma patients whom were diagnosed by ophthalmologists among the actual population of patients, has increased from 8% (2008-2011) to 25% (2017), which still does not meet the needs of the population. Among diagnosed glaucoma patients, the compliance to a consistent regimen is limited to 20.3%. The results clearly indicate that there is necessity for improved awareness and treatment of glaucoma in South Korea.

Advocacy efforts

The Korean Ophthalmological Society (KOS), therefore, has been conducting vigorous campaigns to screen glaucoma and other retinal diseases through fundus image analysis. In addition, it has been putting its utmost efforts to emphasize the importance of glaucoma screening through series of public hearings at the National Assembly, providing political implications to the lawmakers.

An evidence-based approach with the epidemiological data and efforts to make changes through application of proper policies will dramatically reduce the blindness caused by glaucoma, which has been one of the foremost neglected eye diseases in Korea. The processes and results of such efforts will be disseminated to other states and organizations through the International Agency for the Prevention of Blindness (IAPB).

한국에서는 2008년부터 국민건강영향조사(Korean National Health and Nutrition Examination Survey: KNHANES)를 통해 안보건과 관련된 역학자료를 수집하여 발표하고 있다. 그리고 이와 관련한 다수의 학술 논문을 발표함으로써 근시율이 높은 하나의 인종으로 구성된 인구집단에서의 여러 안질환의 유병률을 제시하고 있다. 이러한 발표의 결과들은 증거 중심의 접근법의 중요성을 강조하며, 일반 대중을 대상으로 하는 안보건에 대한 인식개선에 기여하고 있다.

한국은 안보건에 대한 접근성은 매우 높은 나라이다. 예를 들어, 국가 안보건현황의 중요 지표인 백내장 수술률(Cataract Surgical Rate: CSR)의 경우 2005년 2,762건에서 2017년 10,000건 이상으로 증가하였다. 이에 반해, 녹내장에 대한 인식은 여전히 낮은 편이며, 충분한 치료가 제공되고 있지 못하고 있다.

그간 국민건강영향조사의 자료를 통해 녹내장과 기타 망막질환의 유병률과 관련한 지식의 영역을 확장시킬 수 있었다. 2017년부터는 더 정확한 녹내장 유병률 파악을 위해 기존의 안압검사 (Intraocular Pressure: IOP)와 안저사진 분석에 더해 험프리 주변시야계법(Humphrey Perimetry)과 광간섭단층영상(Optical Coherence Tomography: OCT)가 추가 되었다. 이를 통해 얻어진 가장 최근의 조사인 2017년 자료에 따르면 녹내장 의심환자(Glaucoma Suspect)는 6.1%, 고안압증(Ocular Hypertension)은 3.7%, 그리고 원발개방각녹내장 (Primary Open Angle Glaucoma: POAG)가 3.3%로 나타났고, 이들 중 대부분의 안압이 21이하였다.

실제 녹내장 환자 중 실제 안과의사에게 진단을 받은 환자로 나타나는 인식수준의 경우 8% (2008-2011)에서 25%(2017)로 증가하였으나, 이 수치는 여전히 인구집단의 미충족의료를 해결해주지 못한다. 또한, 백내장 진단을 받은 환자 가운데, 치료를 꾸준히 받고 있는 비율은 20.3%로 녹내장 치료에 대한 인식 개선이 필요한 것으로 보인다.

이에 대응하여 한국안과학회 (Korean Ophthalmological Society: KOS)는 안저이미지 분석을 통한 녹내장 및 기타 망막질환에 대한 검사에 대한 캠페인을 진행해 왔으며, 이와 더불어 국회 공청회 등을 통해 정책입안자들에게 정책적 함의를 제공함으로써 녹내장 진단의 중요성을 강조하는데 노력을 기울이고 있다.

역학자료를 활용한 증거기반의 접근방법과 적절한 정책의 적용을 통한 개선을 위한 노력은 한국에서 가장 소외되고 있는 안질환 중 하나인 녹내장으로 인한 실명을 비약적으로 줄일 것이라 예상된다. 이를 위한 과정과 결과는 국제실명예방기구 (International Agency for the Prevention of Blindness: IAPB)를 통해 타 국가 및 기관들에 공유될 것이다.
